Assessment and Progress Monitoring Plan Checklist

Be sure to include the following action steps in your Assessment and Progress Monitoring Plan:

  • Based on your goals (determined in Phase II, Key Action Step 2.1), determine which diagnostic assessments to use.
  • Determine if those same assessments can also serve to monitor student’s progress over time, and if other assessments are needed, select appropriate progress monitoring assessments.
  • Determine logistics:
    • who will administer diagnostic and progress-monitoring assessments
    • when and how often they will be delivered.
  • Organize progress monitoring tools so they are easily accessible to all users.
  • Develop the decision-making process for how student progress will be assessed and guidance for tutoring sessions developed.
  • Train appropriate staff on their role in assessment and progress monitoring.
  • Develop systems to communicate student progress with educators, tutors and caregivers.
